1. Photo ID and Social Security Card
2. Proof of income. Last two pay stubs; proof of social security income , disability, or other government income, proof or retirement or trust fund account; etc. Self-employed must bring copies of the past three tax returns

There is a cost of 53.95 when submitting your application ($33.95 for credit check + $20 for document screening). Document screening will be done using sophisticated software that can detect fraudulent documents created with Adobe Photoshop.
