Awesome Bay View single family home close to Lake Michigan and Downtown. Open concept kitchen/living area, large bedroom and bathroom on the main level & full upstairs loft bedroom with a half bathroom. The home has a full basement and off street parking for two cars.

1. Applicant(s) must be at least 18 years old. Each adult applicant must complete one application. If application
contains any omissions, it will not be processed.
2. A non-refundable application fee of $25 per person is due with application.
3. A valid copy photo I.D.(driver's liscense) must be submitted by each applicant at time of application and
lease signing.
4. Income: Applicant must provide a current paystub for verification. If self-employed, applicant must provide
tax records and/or bank statements. Proof of child support, retirement benefits, disability benefits, rent assistance,
and/or full-time student status is required. Applicant(s) must earn and/or receive forms of income that equal to 3
(three) times the monthly rental amount. If not currently employed, applicant must have good credit history, good
rental history, and a lease guarantor or double security deposit.
5. Residence history: References from two recent lessors will be verified. Applicants may be denied for any of the
following:
a.) Any record of disturbance to neighbors.
b.) Destruction of property.
c.) Living/housekeeping habits which adversely affected the health and welfare of other tenants.
d.) Unauthorized occupants and/or pets.
e.) Eviction for cause.
f.) Criminal activity.
g.) Money owed to landlords/mortgage companies, unless paid prior to move-in.
6. Credit history and criminal background check : A credit report will be obtained and a criminal background
check will be conducted. Applicants may be denied for any of the following:
a.) Current bankruptcy. (Proof must be shown that bankruptcy is closed.)
b.) Falsification of information contained on this application (All discrepancies must be explained
satisfactorily.) Misrepresentations found after a rental agreement is signed will result in a
termination of that rental agreement.
c.) Felony convictions, drug-related offenses, convictions for the possession or distribution of a manufactured or
controlled substance, or a misdemeanor for crime against a person.
7. Occupancy standards: Occupancy is not to exceed two occupants per bedroom or current HUD occupancy
standards or local ordinances. No more than three (3) unrelated adults are permitted in an apt., per City of
Milwaukee ordinance.
8. Pet policy: No exotic animals. Landlord reserves the right to refuse certain types of pets on premises. Tenants
found with unauthorized pets on the premises may result in an eviction.. Pet deposit if required is non-refundable.
9. Security deposit: Deposit is equal to one month's rent, unless specified. Security deposit is fully refundable at end
of lease, minus any amounts legally withheld for damage, unpaid rent, and/or amounts treated as rent. Security
deposit cannot be substituted for any month's rent payment.
10. Guarantors: Lease guarantors must submit an application and meet all applicant criteria.
11. Faxed copies and signatures of these documents are as legally binding as the original documents
