You are desperate!
Your Credit or Finances, or both, will not allow you to go the conventional route:
You need the Seller to help you out!

The Seller will know it, and you are going to pay dearly for this service:
There aren't too many altruistic Sellers out there.

There is no FORM printed by anyone; there are just too many variables.
The terms that can be written into a Lease/Option can be dangerous to you:
How long is the Option period?
How much money are you putting in to the Option?
What happens if you are not able to execute the Option?
How do you know what your financial situation will be 2-5 years from now?
How much is the rent in the meantime?
Who will be responsible for maintenance and repair in the meantime?
What will be the Market Value of the home in 2-5 years?
What will be the Selling price 2-5 years from now?

This is the Ultimate Caveat Emptor!

Landlords each have their own criteria for what they consider is acceptable credit. Some will see a low credit score and reject you out of hand. Others will look at your recent history of renting (get references) and a letter you should prepare explaining your circumstances, why your credit was damaged and what you've done since then to resolve it and give you fair consideration.
If you have a difficult time, consider offering a larger damage deposit or a few months rent up front.

If you are looking to buy I recommend you start looking for a realtor about three months before your arrival. In my experience it takes about a month of viewing homes to find the right home for you; then after a contract is negotiated on the a house you love, it takes from four to six weeks to get all your financing in order and close and fund on your new house. It is also highly recommended that you try to get a preapproval before you start house hunting; you may go to your bank or ask a realtor like myself to recommend a good mortgage company.

Can you be denied insurance, YES, and Insurance Company can deny you coverage, they have that right. Would it be because of previous claims, YES, it could.
If there was a perpetual problem with pipes bursting, they will want some assurance that the problem has been fixed. Insurance Companies do not like to pay for pre-existing conditions.

Are you trying to get an Insurance Company to pay for up-grading your house?
It doesn't work that way.
