My son was going to purchase a house and paid for city inspection. He backed out of the contract due to excessive repairs needed.

If someone else wants to purchase that house, do they have to pay again for a city inspection?

Not necessarily. The city inspection is a report registered with the city and is usually valid for 6 months up to 1 year, based on the city it's conducted in.
This is why most sellers pay for this original city inspection for the C of O and let the buyer assume responsibility for completing items as required prior to obtaining occupancy.
