Good Afternoon Mr. Martin,
I will repeat the answer that was provided to you by Mr. Alvarez, DO NOT, purchase a blank form and complete this yourself, as this has the potential of causing you all sorts of problems when it comes to your title, not to mention any potential tax consequences in the event something should happen to you. How a purchaser obtains title to real estate can have many consequences that you may not realize at this time. It is well worth the time and money to have a real estate attorney provide you with the legal advise on how best to include your wife on title, as well as speaking with an accountant to know how this may affect you on financing basis. All too often homeowners do these changes to their title, and when it comes time to sell the home, it can create all sorts of problems with the title as well. There is a lot to consider, and I would encourage you to seek the proper advise from those that are specialized in this area.
