If the closing is so far out that you will not be moved and settled before having your surgery, you should try to get an extension of closing. If the buyer is getting a loan, it may depend on where they are in the process as to whether they can extend and how long
Obviously, if you need emergency surgery that cannot wait until after your sale and relocation, you may want to consider the rent back option. Otherwise, you may need to speak to a real estate attorney to find out what recourse the Buyer would have if you did not move forward with the transaction.
It seems to me that you should be able to work this out.

Like many communities in the area the community of Riviera Beach is sought after for those shopping in the area. In fact over the past 12 months there have been 47 sales in the community ranging from $25,000 up to $57,000 with a median price of $40,000. Mainly three bedroom townhomes you will find some with two bedrooms and some have garages while others do not.
There are another 18 units under contract in the community; many of these are short sales waiting for short sale approval. As of this morning there are only 7 active listings so in essence there is less than a 2 month inventory which is very, very, very low. So to answer your question based on these numbers the community of Sonoma Bay is highly valued in todays market.
Buyers in todays market must be prepared to move quickly and make aggressive offers or they will be left on the outside looking in as the market continues to recover. Any buyer should by working with a professional Realtor® who can not only identify properties that match the clients goals but also know how to get an offer accepted.

Punta Gorda is a small city on the Southwest coast of Florida. We have everything you could possibly want to do in your retirement.
