Do San Franciscans really use the trolley or is it primarily aimed at tourists?

The cable cars? Yes, they are used as commuter transit by people who happen to live along those lines. The fare for them is included in the monthly Fast Pass, so it's no more expensive for daily riders to take them than to take other transit options, even though the cash fare is much higher.

A "trolley" is actually more like a bus. Most people just refer to them as "electric buses," but the SFMTA officially calls them trolley buses:

I've used it! If you live on California Street near Nob Hill and/or on Van Ness, the trolly is a really cool way of getting to work in the Financial District (California line) and, if you use your MUNI card, it should be the same price as a bus. But yes, you can commute on a trolley!
