if HUD or Title extended the deadline then fees will not be applied, look at the extension release/approval form to see which check box has been marked in there. If you asked for it then you will have to pay. Ask your agent to send an email to HUD and Title to inform them that you are ready to close ASAP to avoid any further accumulation of extension fees.
I currently have a property with HUD , on 2nd extension (15+15 days) passed the original 45 days already. Man ... HUD is slow these days. May have to have a 3rd extension.
