What happens to a quit claim deed if a house is refinanced and the quit claim deed has never been filed with the county?

Great question! If the QCD was never recorded then the general public including the mortgage company doing the refi doesn't know about it and won't find it on a title search. However, that QCD can still be recorded at any given time (like after the property has been refinanced). So really to answer your question directly, nothing "happens" to the QCD but the title of the property will have another mortgage on it so if the owner(s) go to sell the property (including anyone on the QCD even if their name is not on the loan) then that new mortgage will have to be paid at closing.
