I recently purchased a home in Mesa, AZ using an FHA loan. Does the state or city have a Mortgage Insurance Credit Certificate program?

You would get that from your current mortgage company. The City or State are not responsible for that information. You will receive a IRS form 1098 from your Mortgage Company or the Loan Servicing Company.

You should ask your lender that took care of the financing on your purchase. He or she would be able to help you with this.
