Does "address not disclosed" mean foreclosure or pre-foreclosure?

Trulia feeds off of Multiple Listing.
Multiple Listings gives the broker a choice of disclosing the address of the property on the internet. If the broker chooses not to do that then "address not disclosed" shows up.

Not necessarily. Many real estate websites use forms of sydication to obtain listings. They pull info from the MLS listings, other websites, franchise companies, etc. Sometimes when the address is not indicated in a form that the computer can understand (for instance, using abbreviations such as PR for private road), the listing will simply read, "address not disclosed." If you are looking for foreclosures in a certain area, your best bet is to visit a website like the one below, search in your state/county, and find out which real estate agent or agents are handling the most foreclosures in your area. Generally, they will be the most knowledgeable about the local foreclosure properties.
